What happened

Secretary of State Marco Rubio signed a Health Framework of Cooperation with Kenyan President William Ruto and Prime Cabinet Secretary Musalia Mudavadi at the Department of State on December 4, 2025, describing it as “the first signing that we’re going to have with the first country in the world on our America First Global Health Strategy,” according to the State Department broadcast.

Rubio used the ceremony to lay out the strategy’s break with the old model. For years, he said, the United States would promise a country health help, then pay an American or international NGO to run a parallel program — overhead took a cut, the host country “had very little influence,” and only a percentage of the money reached patients. “We are not going to spend billions of dollars funding the NGO industrial complex,” he said; instead the money will build the partner’s own health infrastructure so that in five to eight years countries can sustain their systems themselves. As he stated the figures, the United States will invest $1.6 billion in health assistance in Kenya over the next five years, and Kenya will contribute $850 million of its own: “This is truly a partnership.”

He also thanked Kenya for its role leading the security mission in Haiti for the better part of two years, saying Haiti would be “indescribable” without Kenya’s efforts, and urged other countries — especially in the Western Hemisphere — to contribute money and personnel to the transition to a gang suppression force.

President Ruto said the United States has invested over $7 billion in Kenya’s health over more than a quarter century, credited the partnership with saving millions from malaria, HIV/AIDS and tuberculosis, and said the framework supports his universal health coverage drive — hospital equipment, health commodities, workforce and insurance for all — plus disease surveillance and emergency preparedness. He put Kenya’s own domestic health mobilization at $3 billion and its community health promoters at 107,000, and confirmed Kenya will stay engaged in Haiti through the Gang Suppression Force transition.

Why it matters

This is the America First Global Health Strategy’s founding document: the first country signed, the NGO-bypass model stated by the Secretary himself, and the dollar figures — $1.6 billion American, $850 million Kenyan — on the record from both principals. Every later framework signing will be measured against the Kenya template.
