What happened

The Centers for Medicare & Medicaid Services presented its ACCESS model at an agency event on December 4, 2025, recorded in full on the CMS channel. ACCESS — Advancing Chronic Care with Effective, Scalable Solutions — is, in CMS’s description, a new Innovation Center model that for the first time gives Original Medicare providers and patients access to high-value, technology-supported care options to better prevent and manage chronic disease. CMS’s readout of the convening records that Deputy Secretary Jim O’Neill and CMS Administrator Dr. Mehmet Oz joined leaders from across government, including the Food and Drug Administration, and health care to build momentum for the model.

The model introduces an outcome-aligned payment option that rewards results rather than required activities, according to CMS, enabling new ways of delivering technology-supported care for conditions including high blood pressure, diabetes, chronic musculoskeletal pain, and depression. CMS’s readout states the model would begin in July 2026.

Why it matters

ACCESS is the payment-side expression of the technology agenda Oz laid out at his confirmation hearing: Medicare historically pays for activities, and this model pays for outcomes in the chronic conditions that drive most Medicare spending. It is one of the named, dated CMS models — alongside GENEROUS and BALANCE — whose results this chronicle can check against its stated design.

Scoreboard watch

This is a candidate item for the weekly scoreboard: the ACCESS model’s July 2026 start and its outcome-aligned payment design, as CMS stated them at the unveiling, to be checked against participation and published results as the model runs.
